Refractometer weirdness

For the second brew day in a row, the final numbers from my refractometer have been weird. All along the mashing and brewing process it's spot on. But when I take a final reading as I go into the fermenter, I get one number and 20 minutes later, another. Both times the numbers were .005 off.
The second number has been what the software projected but I'm not sure why it gives me an odd reading the first time. It's calibrated and other than this, it seems to be working ok.
Anyone else see this?

Only 6 oz in the boil. Last one was only 4 and there was nothing but H2O whilst brewing. That's the truth!

Sounds like stratification...

If you are taking it in the boil pot and then the fermenter, stir first...

Same if you are taking them both in the fermenter...

(You said while transfering, so I don't know...)

It took me a couple measures to realize I was trapping some sanitizer in my syringe. That threw my numbers off a few times.
